The Passport Center is located in CSUDH College of Continuing and Professional Education (CCPE) EE-1201, just east of the Dignity Health Sports Park.

CSUDH Passport Assistants  Parking: Important Information

You can park in Lots 3 (closest) or 6 (west of LaCorte Hall/Sculpture Garden). There are a limited number of 30-minute visitor spots near the Continuing & Professional Education complex; however, BE PREPARED for longer stays. If your visit requires more than 30 minutes, we recommend you purchase a daily pass ($10) to avoid getting ticketed. (There are parking permit kiosks in the corners of Lots 3 and 6.)

Parking is STRICTLY ENFORCED in all lots. Please make sure to display a daily permit or current term parking permit as required. Should you have any question(s) please contact Parking Services at (310) 243-3725. Street parking may also be available along Victoria Street, and Continuing & Professional Education is a minimal walk from the Gate D entrance.

Parents of small children: please be aware that our office building also houses research and academic staff, and ask that children not be allowed to run/play inside the building; we have a friendly, park-like open seating area in front of the CCPE Classroom Building.

 Renewing Your Passport?
Please note that passport renewals do not require a visit to a Passport Acceptance Facility. Renewals can be mailed to the National Passport Processing Center.

  Applying for a Passport: Quick Checklist

To apply for a United States Passport, you will need to follow these steps:

  1. Complete Form DS-11: Application For A U.S. Passport
  2. Provide Original Identification Document(s)
    (Photocopies can be made by us for a fee)
    • Evidence of U.S. Citizenship
      Certified Birth Certificate/U.S. Naturalized Certificate (copies are not acceptable)
    • Official Identification
      Photo ID (Driver's License or Military ID)
  3. Provide One (1) Passport Photo
    (Photo services are available onsite for a $15 fee)
  4. Pay the Applicable Fees (2 Separate Payments)
    • Payment #1: Check or Money Order ONLY to U.S. Department of State
    • Payment #2: Check, Money Order, Debit/CC (No Cash) to CSUDH Foundation
      (see fee chart below)
  5. Submit completed Form DS-11 & documentation in person
    Visit the CSUDH Passport Hub Center during office hours
    (by appointment only)

Important Note: Do NOT submit any passport application information to our office via e-mail. We cannot accept applications via email. You must bring your application to the Passport Hub Center in person.

Passport Application

Form DS-11 - U.S. Passport Application:

  • Applying for a U.S. passport book or card for the first time
  • Current U.S. passport is valid but has been lost, stolen or damaged
  • Previous U.S. passport was issued more than 15 years ago

Passport Renewals

Form DS-64 - U.S. Passport Renewal Application for Eligible Individuals:

  • Used to report the loss or theft of a valid U.S. passport book or card
  • Submitted with Form DS-11 and supporting documents to replace a previously lost or stolen passport
  • Used alone to report the loss or theft of a passport even if you do not want a replacement passport

Passport Renewals by Mail

  Please note: Passport renewals do not require a visit to a Passport Acceptance Facility. Renewals can be mailed directly to the National Passport Processing Center, Post Office Box 90155, Philadelphia, PA 19190-0155. However, if you prefer/require assistance with a renewal, we will charge our standard processing fee for service.

Form DS-82 - U.S. Passport Renewal Application for Eligible Individuals (By Mail):

This form is to renew an existing, 10-year passport, or replace a limited-validity passport that is more than 1 year old. To qualify for a renewal:

  • You must be in possession of your previously issued passport book/card

  • Passport book/card must have been issued less than 10 years ago

  • Passport book/card must have expired less than 5 years ago

Other requirements:

  • Only use if the name on the application and the previous passport are the exact same (unless you are submitting a legal name change document or marriage certificate).
  • The previously issued, undamaged passport book/card must be submitted with Form DS-82 (passport renewal application) by mail.
  • In order to protect the contents of your mailing from the elements throughout the delivery process, it is strongly recommended that you mail your passport application and personal documents in an envelope large enough to fit the application without folding. Please do not use a standard letter-size #10 (4 1/8" x 9 1/2") envelope.

Passports for Minors

Form DS-3053 - Statement of Consent: Issuance of a U.S. Passport to a Minor Under Age 16:

  • Minors under age 16
  • Completed by the non-appearing parent
  • Must be notarized within the last 90 days, and must be accompanied by a copy of the non-appearing parent's photo ID (front and back)
  • Submitted with Form DS-11

Passport Name Changes

Form DS-5504 - Application for a U.S. Passport - Corrections, Name Change within 1 Year of Passport Issuance, and Limited Passport Holders:

  • To request a name change
  • To correct data errors (typos)
  • To replace limited-validity passports (if eligible)
  • You may submit Form DS-5504 by mail
